Make a referral
Referrals are welcome from participants, families, guardians, Support Coordinators, plan managers, providers and health professionals.
To help us respond quickly, please include the information below in your referral.
What to include
- 1The participant's name, date of birth and contact details
- 2Their NDIS number and plan dates, and how the plan is managed
- 3The services you are seeking: Specialist Behaviour Support, Early Intervention, Therapeutic Services or training
- 4A brief description of current concerns and goals
- 5Contact details for the guardian or decision-maker, if applicable
- 6Any relevant reports, such as previous Behaviour Support Plans or assessments
